A ingestão diária recomendada de ferro em uma mãe que amamenta é de 9-10 mg e não se deve ultrapassar 45 mg diários (NIH 2023, 2022, 2001). A suplementação pode diminuir a concentração de cobre e zinco no plasma e leite materno. (Mello 2013, Haidar 2005, Ortega 1998) A anemia materna deve ser prevenida e tratada, mas nem a doença nem seu tratamento contraindicam a amamentação. Há uma alta prevalência de anemia nutricional em mães que amamentam (Shashiraj 2006), que chega a ser de 47% em mães de baixo nível socioeconômico. (Bodnar 2001) Durante a gravidez e a amamentação há uma transferência de ferro de mãe para filho que tende a evitar tanto a infecção mediada por ferro e o estresse oxidativo quanto o risco potencial de deficiência de ferro materno e infantil. (Miller 2016) A perda de sangue durante o parto é um fator importante de anemia pós-parto (Chan 2001). A gravidez durante a amamentação aumenta o risco de anemia (Shaaban 2015). Por outro lado, a amamentação prolongada é um fator protetor de anemia nas mães que amamentam. (Lakew 2015) A anemia por carência de ferro aumenta o risco de depressão pós-parto (Azami 2019) e é um fator de risco de abandono precoce da amamentação (Rioux 2006), possivelmente por sensação (real ou não) de leite insuficiente. (Henly 1995) Segundo alguns autores, há correlação entre a hemoglobina materna e a do bebê que mama alimentado de forma exclusiva nos primeiros 6 meses de vida (Marques 2016, Teixeira 2010), por isso é necessária a prevenção de anemia pré-concepcional, durante a gravidez e a amamentação. (Teixeira 2010) Quando a anemia materna se associa a amamentação prolongada há mais risco de anemia no bebê que mama em países em desenvolvimento (Meinzen 2006). Mas outros trabalhos demonstram que os níveis de ferro e lactoferrina no leite são independentes dos níveis de ferro e hemoglobina plasmáticos da mãe (Marques 2016, Shashiraj 2006, Murray 1978) e que os bebês que mamam com amamentação materna exclusiva têm níveis de ferro plasmático normais independentemente do estado de ferro da mãe. (Murray 1978) O ferro é excretado em pequena quantidade no leite materno por um mecanismo autorregulado. Não há correlação entre a ingestão diária de ferro por parte da mãe e sua concentração no leite materno (Keikha 2021, Yalçin 2009, Hannan 2009, Muslimatun 2001). Os suplementos de ferro para a mãe não aumentam os níveis de ferro no leite materno nem no plasma do bebê que mama de forma significativa. (Breymann 2007, Baykan 2006) A anemia perniciosa materna por falta de absorção de vitamina B12 ou por dietas deficitárias como a vegana pode provocar anemia e sintomas físicos e/ou neurológicos graves no bebê que mama.
