Comentário
É um antipsicótico atípico utilizado no tratamento da esquizofrenia, do transtorno bipolar, do transtorno depressivo maior, da irritabilidade associada com o autismo e da síndrome de Tourette. Administração oral (ou intramuscular) uma vez ao dia. Seus dados farmacocinéticos (baixa biodisponibilidade oral, amplo volume de distribuição, peso molecular moderadamente elevado e alto percentual de ligação a proteínas plasmáticas) explicam a ínfima ou nula passagem para o leite materno observada (Nordeng 2014, Watanabe 2011, Lutz 2010, Schlotterbeck 2007). Não foram observados problemas em bebês cujas mães o tomavam (Nordeng 2014, Tarikh 2014, Lutz 2010). Os níveis em plasma desses bebês foram indetectáveis ou muito baixos (Watanabe 2011). É controvertida sua relação com a prolactina, pois diminui os níveis de prolactina e foi empregado para tratar prolactinomas e hiperprolactinemias induzidas pelo uso de outros antipsicóticos (Propst 2016, Bakker 2016, Ranjbar 2015, Raghuthaman 2015, Safer 2013, Byerly 2009, Wolf 2007, Lorenz 2007, Goodnick 2002) e foram publicados vários casos de diminuição da produção de leite (Komaroff 2021, Walker 2019, Yskes 2018, Morin 2017, Frew 2015, Mendhekar 2006), embora haja também casos de hiperprolactinemia/galactorreia desencadeados pelo aripiprazol (Gunes 2018, Sogawa 2016, Prost 2016, Saraf 2014, Mendhekar 2005, Ruffatti 2005). Autores expertos consideram seguro o uso desta medicação durante a amamentação (Hale, Cuomo 2018, Larsen 2015, Parikh 2014). Convém vigiar possível aparição de sonolência no bebê que mama (Hale 2017 p. 74). Dada sua lenta eliminação e o risco de afetar a produção de leite, pode ser valorável o utilizar outros antipsicóticos compatíveis de menor meia-vida, especialmente durante o período neonatal e em caso de prematuridade (Uguz 2021 e 2016).
