Anticorpo monoclonal humano de IgG1λ. Indicado no tratamento do lúpus eritematoso sistêmico. Administração intravenosa a cada 2 a 4 semanas. Seu elevadíssimo peso molecular explica o ínfimo passe ao leite materno observado (Higuchi 2025, Blomjous 2024, Saito 2024 y 2020) e calculado (Stratigakis 2023), já que as moléculas de mais de 800-1.000 Da. têm dificuldades para se excretarem no leite materno. (Hale, Almas 2016, Anderson 2016). A concentração no leite materno é 200 a 500 vezes inferior à do plasma materno. (Stratigakis 2023, Saito 2020). Não foram observados problemas em bebês de mães tratadas com belimumab. (Oaklander 2024, Saito 2020, Danve 2014) Por sua natureza proteica é inativado no trato gastrointestinal, não sendo absorvido, (biodisponibilidade oral praticamente nula), o que dificulta ou impede o passe ao plasma do bebê a partir do leite materno ingerido (Lactmed, Hale, Anderson 2021), salvo em prematuros e período neonatal imediato, nos quais pode haver maior permeabilidade intestinal (Sammaritano 2020). Esperar duas semanas após o parto antes de retomar o tratamento permite que os níveis plasmáticos do recém-nascido desçam dos anteriores intraútero e também reduz ao mínimo a transmissão ao bebê. (Krysko 2023) Autores especialistas e associações médicas de reumatologia consideram compatível seu uso durante a amamentação.
