Comentário

O lítio é eficaz no tratamento da mania, do transtorno bipolar e da depressão unipolar recorrente. É preciso monitorar a concentração sérica, já que a margem entre a concentração terapêutica e a tóxica é estreita (0,5-1,2 mmol/L). Administração oral em duas doses diárias. É excretado em leite materno em quantidade moderada (Imaz 2019, Bogen 2012, Viguera 2007, Moretti 2003, Schou 1973), que poderia ser clinicamente significativa por exceder frequentemente 10% da dose relativa. Podem alcançar-se níveis plasmáticos no bebê até 10%-60% dos terapêuticos na mãe. (Gehrmann 2021, Imaz 2021(set e jun), Imaz 2020, Marín 2018 e 2011, Khan 2016, Frew 2015, Bogen 2012, Viguera 2007, Moretti 2003, Skausig 1977, Sykes 1976, Schou 1973, Tunnessen 1972) A maioria (≈80%) dos bebês cujas mães tomam lítio não apresentam problemas clínicos nem de crescimento ou desenvolvimento neurológico, nem a curto nem a longo prazo. (Gehrmann 2021, Imaz 2021(set e jun) e 2020, Marín 2018 e 2011, Frew 2015, Bogen 2012, Tanaka 2008, Moretti 2003, Viguera 2007, Sykes 1976) Autores especialistas consideram possível o tratamento com lítio durante a amamentação realizando periodicamente controles clínicos, de peso e analíticos (lítio, TSH, função renal) na mãe e bebê. (Uguz 2021 e 2016, Imaz 2020, Rybakowski 2019, Petzold 2018, Pacchiarotti 2016, Khan 2016, Sriraman 2015, Hotham 2015, Grover 2015, Larsen 2015, Rowe 2013, Bogen 2012, Davanzo 2011, Sharma 2009, Rubin 2004, WHO 2002, Iqbal 2001, Moretti 2000, Koren 1999, Austin 1998, Villeneuve 1989), o que nem sempre vai ser possível. (Galbally 2018) Outros autores o desaconselham. (Newmark 2019, Poels 2018, Hermann 2019, Even 2006, Schou 1973) Durante a primeira semana, em recém-nascidos ou prematuros e em bebês desidratados ou com infecção, que podem ter diminuídos os mecanismos de eliminação do lítio, foram descritos sinais de toxicidade por lítio absorvido com o leite materno: cianose, letargia, hipotonia ou aumento leve e transitório de TSH. (Marín 2011, Viguera 2007, Tunnessen 1972) Amamentar tomando lítio tem menor risco em bebês a termo saudáveis, com mães ou família com suficiente capacidade de monitorar o aparecimento de efeitos adversos, controle médico e com possibilidade de monitorização de níveis de lítio e da função tiroideia na mãe e bebê (Marín 2011, Viguera 2007, Moretti 2003). A medição de níveis de lítio em bebês deve fazer-se com técnica adequada (Tanaka 2008). Há menor risco em monoterapia e se o bebê é maior de dois meses. Alguns autores recomendam suspender ou diminuir a administração de lítio 1 a 2 dias antes do início do parto ou da cesárea programada (Rybakowski 2019, Bogen 2012, Newport 2005) para diminuir os níveis plasmáticos do recém-nascido. Outros autores (Wesseloo 2017, Khan 2016, Bergink 2014) desaconselham esta suspensão e recomendam adequar a dose a níveis de lítio adequados. Após o parto, convém reduzir a dose materna em 50% ou à prévia antes da gravidez (Khan 2016, Bergink 2014, Bogen 2012), em função dos controles maternos de litemia, que devem ser frequentes durante a gravidez e no pré e pós-parto imediato. (Wesseloo 2017, Westin 2017) Embora tenha sido publicado algum caso de galactorreia por lítio (Ohishi 1983), o lítio não costuma alterar os níveis de prolactina e em algum estudo os diminuiu pelo que pode ser um bom protetor frente a outros fármacos que tendem a aumentar os níveis de prolactina. (Pacchiarotti 2015, Lanng 1977) Deve fazer-se um manejo multidisciplinar da mãe e do bebê em colaboração com psiquiatras, obstetras, pediatras e analistas (Austin 1998). A decisão sobre o tratamento com lítio e a amamentação deve tomá-la a paciente, após uma discussão colaborativa com seu psiquiatra, obstetra e pediatra.
