Comentário
É antidepressivo tricíclico com ações e usos similares aos amitriptilina. Tem marcadas propriedades antimuscarínicas e sedantes. Utilizado tratamento depressão e transtorno obsessivo-compulsivo. Administração oral em uma ou duas doses diárias. Excreta-se leite materno em quantidade clinicamente não significativa. (Khachman 2009, Yoshida 1997, Schimmell 1991) e não foram observados problemas em lactentes cujas mães a tomavam. (Khachman 2009, Nulman 2002, Birnbaum 1999, Yoshida 1997, Wisner 1995, Schimmell 1991, Misri 1991) Níveis em plasma ditos lactentes foram indetectáveis ou muito baixos. (Khachman 2009, Birnbaum 1999, Yoshida 1997, Wisner 1995, Schimmell 1991) Devido seu ínfimo passo ao leite, não consegue-se com amamentação evitar sintomas privação em recém-nascidos mães que tomaram medicamento durante gravidez. (ter Horst 2012, Khachman 2009) Foram descritos galactorreia e aumento níveis plasmáticos prolactina. (Cordes 2011, Egberts 1997, Fowlie 1987) Podem ser preferíveis alternativas conhecidas mais seguras, especialmente durante período neonatal e em caso prematuridade. (Sriraman 2015, Gentile 2014, Davanzo 2011, Weissman 2004, Gjerdingen 2003) Listado medicamentos essenciais OMS: compatível com amamentação. (WHO-UNICEF 2002) Mulheres enfermas depressão durante gravidez necessitam mais apoio amamentação por tener mais risco problemas e cessação precoce amamentação.
