Antidepressivo inibidor da recaptação de serotonina e norepinefrina, precursor da desvenlafaxina (ODV), na qual se metaboliza. Administração oral em uma a três doses ao dia. Tanto a venlafaxina como seu metabólito ODV são excretados no leite materno em quantidade pequena-moderada (Pogliani 2019, Schoretsanitis 2018, Rampono 2011, Newport 2009, Misri 2006, Koren 2006, Berle 2004, Ilett 2002), que pode alcançar 10% da dose relativa. (Newport 2009, Ilett 1998) Os níveis no plasma dos lactentes cujas mães a tomam são de indetectáveis a baixos (Rampono 211, Newport 2009, Berle 2004, Weissman 2004, Hendrick 2001, Ilett 2002 y 1998) , às vezes entre 10% (Berle 2004) e 37% (Newport 2009) dos níveis maternos. Há numerosos estudos que demonstram a ausência de efeitos secundários a curto e longo prazo em lactentes cujas mães tomam venlafaxina, nem em nível de desenvolvimento físico, nem psicomotor. (Pogliani 2019, Rampono 2011, Newport 2009, Misri 2006, Berle 2004, Hendrick 2003 y 2001, Ilett 2002 y 1998) Um recém-nascido cuja mãe tomava doses altas (300 mg/dia) de venlafaxina desde o segundo trimestre da gravidez apresentou letargia e sucção pobre que melhorou ao suspender a amamentação materna. (Tran 2016) A má adaptação neonatal que pode se apresentar após a dose de inibidores seletivos da recaptação de serotonina durante a gravidez (Hale 2010, Tran 2016) pode ser atenuada se os recém-nascidos forem amamentados (Kieviet 2013, Lanza 2009) . Dois recém-nascidos cujas mães tomavam durante a gravidez 300 e 375 mg ao dia de venlafaxina apresentaram sintomas de abstinência nos primeiros dias que melhoraram ao se instaurar a amamentação materna. (Boucher 2009, Koren 2006) Pode produzir galactorreia e níveis elevados de prolactina.
