Há dois tipos de vacinas contra a bactéria Bordetella pertussis, causadora da coqueluche ou tosse ferina: vacina de células inteiras: Pertussis inteira, Pe. Contém Bordetella pertussis inativada. Vacina acelular: Pertussis acelular, Pa. Contém fragmentos proteicos da bactéria e pode conter toxina de pertussis. Ambas são administradas combinadas com as vacinas contra difteria e tétano e também com outras vacinas. Após a vacinação, aparecem níveis no leite materno de IgA contra o toxoide pertussis (Halperin 2011). Os níveis de anticorpos contra a coqueluche no colostro foram significativamente mais altos entre mulheres vacinadas durante a gravidez em comparação com mulheres não vacinadas. (Orije 2021, Abu 2014) A amamentação protege contra a coqueluche, diminuindo o risco de adoecer. A proteção é maior se a amamentação for exclusiva e se a mãe tiver sido vacinada durante a gravidez. (Nascimento 2021, Curtis 2017) Diversas sociedades médicas e autores especialistas consideram seguro o uso desta vacina durante a amamentação. (CDC 2023 y 2011, Red Book 2021-2024 p108-9, CAV-AEP 2019, Taylor 2003, Briggs 2015, Sachs 2013, Raney 2012, Chen 2010, WHO 2002) VACINAS E AMAMENTAÇÃO: (RedBook 2021-2024, CDC 2023, 2011 y 2000, Taylor 2019, Sachs 2013, Munoz 2013, Raney 2012, Perin 2012, Gall 2012, Chen 2010, Pisacane 2010, Schmidt 2004, López 2002, Pickering 1998, Pabst 1997, Hahn 1990) As vacinas são compatíveis com a amamentação, tanto as que são microrganismos vivos atenuados como mortos, inativados, formadas por partes ou toxoides deles ou geradas por tecnologia recombinante. Não passam para o leite, salvo a da rubéola, que não costuma infectar o lactente ou o faz de forma leve, e não ocasionam problemas nos lactentes, salvo a da febre amarela em menores de 6 meses. A amamentação pode melhorar a resposta de anticorpos das vacinas e provocar menos efeitos secundários como febre ou anorexia. O pós-parto imediato é o melhor momento para vacinar contra sarampo-rubéola-caxumba e varicela em mulheres que não estivessem imunizadas. As mulheres que amamentam podem e devem ser protegidas com as vacinas recomendadas como os demais adultos. Os lactentes amamentados devem ser imunizados segundo o calendário vacinal correspondente.
