Comentário
Dolutegravir é um inibidor da integrase do VIH. É usado combinado com outros antirretrovirais ( tenofovir + emtricitabina ou lamivudina ) para o tratamento da infecção por VIH e SIDA. Administração oral. É excretado no leite materno em quantidade clinicamente não significativa (Aebi 2022, Dickinson 2021, Waitt 2019, Gini 2018, Kobbe 2016) e não foram observados problemas em lactentes cujas mães o tomavam. (Malaba 2022, Kobbe 2016) Os níveis no plasma desses lactentes foram muito baixos, menores que 10% dos níveis plasmáticos maternos. (Aebi 2022, Dickinson 2021, Waitt 2019, Gini 2018, Kobbe 2016) Uso autorizado em crianças de mais de 4 semanas de vida e peso de pelo menos 3 kg. Durante a gravidez e o pós-parto, os regimes de terapia antirretroviral combinada que incluem dolutegravir (especialmente em associação com emtricitabina e tenofovir alafenamida) parecem mais seguros e eficazes no controle da carga viral que os que não o incluem (Chinula 2023), podendo inclusive associá-lo unicamente à lamivudina.
