Comentário
Antirretroviral análogo de nucleosídeos inibidor da transcriptase reversa (ITIN), com atividade antiviral contra o VIH-1 e o vírus da hepatite B (HB). Indicado no tratamento da infecção por VIH, SIDA e hepatite B. Administração junto a outros antirretrovirais por via oral em uma dose diária. Embora se concentre no leite materno, atinge baixa concentração no leite materno, clinicamente não significativa (Aebi 2022, Waitt 2018 y 2017, Mugwanya 2016, Benaboud 2011), muito inferior (2% a 4%) à dose utilizada em recém-nascidos e lactentes. (Mugwanya 2016). Não foram observados problemas em lactentes cujas mães o tomavam, exceto diarreia leve em 4% (Mugwanya 2016) Os níveis plasmáticos desses lactentes foram indetectáveis ou muito baixos, menores que 20% dos níveis plasmáticos maternos. (Aebi 2022, Waitt 2018 y 2017, Mugwanya 2016) Uso autorizado em Pediatria. A dosagem em maiores de 3 meses é de 6 mg/kg/dia e metade desde o nascimento até essa idade (Ribera 2011). Seu uso em mães que amamentam é considerado seguro, tanto para tratamento do VIH e HB quanto em profilaxia pré-exposição.
