Comentário
Antibiótico macrolídeo. Administração oral a cada 6 a 12 horas. É excretado em leite materno em quantidade insignificante. (Zhang 1997, Matsuda 1984) É muito utilizada em Pediatria, em bebês pequenos, com muito boa tolerância. (Ng 2009) Relacionou-se a exposição temprana direta à eritromicina durante o primeiro mês com o aparecimento de estenose hipertrófica do piloro (Almaramhy 2019, Lund 2014, Maheshwai 2007, Prescrire 2001), mas há controvérsia se também através do leite materno, encontrando-a alguns autores (Lund 2014, Sorensen 2003, Stang 1986) e não outros (Abdellatif 2019, Almaramhy 2019, Goldstein 2009, Ito 1991). Pode ser prudente evitar seu uso durante o primeiro mês de amamentação. Além do primeiro mês, a eritromicina é compatível com a amamentação. Embora infrequente, deve-se considerar a possibilidade de gastroenterite transitória devido à alteração da flora intestinal em bebês cujas mães tomam antibióticos. (Ito 1993). Autores especialistas consideram compatível o uso desta medicação durante a amamentação (Hale, LactMed, Briggs 2015, Butler 2014, Kong 2013, ASGE 2012, Bar-Oz 2003, Chin 2001, Fulton 1992). Lista de medicamentos essenciais OMS: compatível com a amamentação. (WHO-UNICEF 2002) Academia Americana de Pediatria: medicação generalmente compatível com a amamentação materna. (AAP 2001) USO TÓPICO: A eritromicina tópica não tem nenhum risco durante a amamentação. A pequena dose e a escassa absorção plasmática da maioria dos preparados dermatológicos (van Hoogdalem 1998) ou oftalmológicos tópicos tornam pouco provável a passagem de quantidade significativa para o leite materno. Não aplicar sobre o peito para evitar que o bebê que mama o ingira; se necessário, aplicar após uma amamentação e limpar bem com água antes da próxima. Convém evitar a aplicação sobre o mamilo de cremes, géis e outros produtos de aplicação tópica que contenham parafina (óleo mineral) para que o bebê que mama não a absorva. (Concin 2008, Noti 2003)
