Comentário
Barbitúrico de ação prolongada por sua longa meia-vida. Utiliza-se em epilepsia como anticonvulsivo. Administração oral, intravenosa e intramuscular. É excretado em leite materno em proporção muito variável, podendo alcançar concentrações que poderiam ser clinicamente significativas (O'Connor 2005, Shimoyama 2000, Gomita 1995, Meyer 1988, Kaneko 1979). Autores mais recentes consideram que a passagem de fenobarbital para o leite materno não é preocupante do ponto de vista clínico. (Davanzo 2013, Mercadé 2012, Harden 2009) Em plasma de bebês de mães que tomavam fenobarbital mediram-se níveis que superaram os limites do intervalo terapêutico (15-40 µg/ml). (Pediamecum 2025, Pote 2004, Gomita 1995, Kuhnz 1988) Descrita sedação em bebês de mães que o tomavam mas também síndrome de abstinência com espasmos após um desmame súbito, principalmente em recém-nascidos. (Rauchenzauner 2011, Kuhnz 1988, Juul 1969, Knott 1987) Deve utilizar-se com precaução durante a amamentação (Pennell 2006). A monitorar estado de alerta e alimentação suficiente nos bebês. Pode estar indicado monitorizar níveis plasmáticos no bebê, tanto se aparece sedação como para guiar um desmame de forma progressiva. Diversas sociedades médicas e autores especialistas consideram compatível o uso de fenobarbital durante a amamentação com monitorização cuidadosa do bebê, realizando níveis plasmáticos se apresenta signos de sonolência, má alimentação ou escasso aumento de peso. (Crettenand 2018, Veiby 2015, Davanzo 2013, Mercadé 2012, Rauchenzauner 2011, Pack 2006, O'Brien 2005, Moretti 2000) Lista de medicamentos essenciais OMS: compatível com a amamentação. (WHO-UNICEF 2002) Outros antiepilépticos com perfis farmacológicos mais favoráveis são preferíveis durante a amamentação (LactMed, Hale, Rubin 2004, Hägg 2000, Pons 1994), especialmente se a mãe não tomava fenobarbital durante a gravidez.
