O Filgrastim é um fator estimulante não glicosilado de colônias de granulócitos (G-CSF) obtido por tecnologia recombinante. O produto natural é um componente natural do leite materno. O pegfilgrastim e o lipegfilgrastim são Filgrastim conjugado com monometoxi polietilenoglicol (PEG). São utilizados para induzir a produção de granulócitos e reduzir o risco de infecção depois da terapia mielossupressora. Administração intravenosa ou subcutânea. Seu elevadíssimo peso molecular explica a passagem insignificante ou nula para o leite materno observada tanto de filgrastim como de lenograstim. (Nelson 2018, Kaida 2007, Shibata 2003) Sua biodisponibilidade oral praticamente nula (Pessach 2013) dificulta a passagem para o plasma do lactente a partir do leite materno ingerido, já que, por sua natureza proteica, degrada-se no trato gastrointestinal, não sendo absorvido. O Filgrastim não é absorvido por via oral, inclusive durante o período neonatal e em prematuros. (Calhoun 2003) O fator estimulante de colônias de granulócitos foi utilizado em recém-nascidos e em prematuros com fins preventivos e de tratamento de sepse neonatal e enterocolite necrosante, sem que fossem observados efeitos adversos. (Canpolat 2006, Carr 2003, Bedford 2001, Gillan 1994) O fator estimulante de colônias de granulócitos (G-CSF) é um componente normal do leite materno. O G-CSF bovino encontra-se na dieta habitual dos que comem carne. (Pessach 2013) Uma mãe nos referiu em junho de 2017 ter-se injetado filgrastim enquanto amamentava um bebê de 21 meses, que não apresentou nenhum problema, salvo talvez fezes um pouco mais soltas por alguns dias. Autores especialistas consideram seguro o uso desta medicação durante a amamentação.
