Em 1975, antes de haver imunização universal neonatal contra Hepatite B (HB), foi publicada uma frequência de transmissão vertical (mãe-filho) de 53% em lactentes amamentados e de 60% em não amamentados (Beasley 1975), sugerindo que a amamentação não desempenhava papel importante na transmissão vertical de HB. Em 369 filhos de mães com (HB) crônica vacinados ao nascer, não houve nenhum caso de transmissão de HB entre os 101 amamentados e houve 9 casos (3%) de contágio entre os 268 alimentados com fórmula artificial. (Hill 2002) Embora o antígeno de superfície do vírus da hepatite B (HBsAg) seja detectado no leite materno, não há diferenças na frequência de transmissão de mães portadoras de HBsAg para seus filhos amamentados em relação às que não amamentam. (Red Book 2021-24, Zhou 2021) Há acordo generalizado entre sociedades médicas e consensos de especialistas de que a hepatite B materna não se transmite ao lactente através do leite e, portanto, não contraindica a amamentação. (Red Book 2021-24, Zhou 2021, Mammas 2017, Fouquet 2016, Visvanathan 2016, Dionne 2016, Sogni 2015, Dunkelberg 2014, NIHCE-UK 2013, SFP 2013, Bzowej 2012, Borgia 2012, CDC 2010, Bzowej 2010, Fiore 2010, Larke 1977) Antes da vacinação neonatal, nunca foi demonstrada transmissão vertical através da amamentação (Castillo 2017), e tampouco desde que se segue imunoprofilaxia neonatal adequada com a administração de vacina anti-HB e imunoglobulina HB ao recém-nascido de mãe HBsAg + (Xiao 2017, Yi 2016, SMFM 2016, García 2015, Lamberth 2015, Zhang 2014 y 2014, Geeta 2013, Tran 2012, Shi 2011, Pol 2011, Zheng 2011, Sookoian 2006, Pronczuk 2002, CDC 1990, de Martino 1985), inclusive em mães com alta infectividade. (Kumar 2012, Zheng 2011) Vários medicamentos antivirais utilizados na hepatite B não contraindicam a amamentação (Wong 2018, Chamroonkul 2017, Marcellin 2016, NIHCE-UK 2013, Pandhi 2104, Hirnschall 2013, Pol 2011), embora alguns autores recomendem suspender o tratamento enquanto se amamenta. (SMFM2016, Lamberth 2015, Bzowej 2012, Kumar 2012) Convém conversar em cada caso com a mãe devido ao problema ético e legal que supõe a contraindicação frequente de amamentar escrita nas bulas dos medicamentos. (Rosenthal 2015) A aceitação entre profissionais da compatibilidade da Hepatite B e de seu tratamento antiviral com a amamentação foi abrindo caminho pouco a pouco ao longo dos últimos anos. (Ahn 2010) Apesar desse consenso esmagador a favor da compatibilidade, sustentado também pela OMS (Pronczuk 2002), inclusive em casos de não vacinação (Indian Ped 1997), alguns países, como a China, têm taxas muito baixas de amamentação entre mães HBsAg +.
