O Vírus da Imunodeficiência Humana ( VIH ) é um retrovírus que destrói os linfócitos e outras células encarregadas do sistema imunológico (defensivo) das pessoas, possibilitando o aparecimento de infecções e outras doenças de tipo imunológico. A Síndrome da Imuno-Deficiência Adquirida (SIDA) é a etapa final da infecção por VIH, na qual o sistema imunológico do organismo está muito danificado pelo vírus. Nem todas as pessoas com VIH desenvolvem SIDA. O VIH é transmitido através de fluidos corporais: nas relações sexuais; pelo sangue: compartilhamento de agulhas para consumo de drogas, transfusões e outros contatos com sangue de pessoa infectada; de mãe para filho durante a gravidez, parto ou amamentação materna ( LM ). Antes do aparecimento do Tratamento Anti-Retroviral combinado ( TARc ), o risco de Transmissão Materno-Infantil ( TMI ) do VIH através da LM era de 5 a 42% ( White 2014) . A prevenção passa por evitar a LM em regiões ou países (desenvolvidos) nos quais o uso de leite de fórmula é Aceitável, Factível, Acessível, Sustentável e Seguro (AFASS). Caso contrário, a LM de mães com VIH é preferível por ser maior a morbimortalidade derivada da alimentação com substitutos da LM. (García 2015) A eficácia do TARc comprovada em experiências com mães amamentando na Nigéria e Zâmbia com TMI ≈ 0 (Ngoma 2015, Okafor2014) , similar à obtida (< 2%) quando se evita a LM (Lolekha 2017) , faz com que desde 2010 a OMS tenha evidência suficiente para recomendar como opção manter a LM em mães com VIH, sempre que se assegure: seguimento e continuidade do TARc por toda a vida (Opção B+); controles de carga viral mensal (< 50 copias/ml); extração e aquecimento da LM em casos especiais. Desde 2016, as recomendações de amamentação da OMS para mães VIH que desejam amamentar são as mesmas que para as mães não VIH (amamentação exclusiva 6 meses e depois parcial dois ou mais anos), sempre que tomem o TARc e sigam com os controles de carga viral. A exclusividade ou não da amamentação e sua duração não influem na TMI. As autoridades sanitárias fornecerão o TARc e os controles e, em função das possibilidades do país, decidirão se desaconselhar a LM como medida preventiva. (WHO/OMS 2016) Dada a efetividade do TARc, que reduz a carga viral a indetectável ou menos de 40 copias/mL e evita a transmissão, o lema da campanha Indetectável = Intransmissível ( I = I ) impôs-se como paradigma na transmissão por via sexual. (Rodger 2019 y 2016, The Lancet HIV 2017) Mas há dúvidas sobre se I = I é válido para a transmissão através da LM (Kahlert 2018, Waitt 2018) , já que o TARc suprime o RNA do VIH do leite, mas não o vírus (o DNA) associado a células T CD4+ que há na LM (Prendergast 2019, Van de Perre 2012) . No entanto, sabe-se que, com DNA-VIH em secreções vaginais e carga viral nula, não há contágio horizontal. (Nelson 2020, Prazuck2013) Em revisões sistemáticas não se identificou nenhum caso de TMI do VIH através da LM em um marco definido de “cenário ótimo” (Kahlert 2018) : mulher grávida que tem adesão à dose de seu TARc, atenção clínica regular e carga viral de VIH de <50 copias de ARN/ml durante a gravidez e a amamentação . A recomendação geral em países desenvolvidos de penalizar ou proscrever a LM em mães com VIH pode já não estar justificada; defende-se um processo de tomada de decisões compartilhado e de apoio a mães com VIH que decidem amamentar, assegurando a dose do TARc e os controles de carga viral. (Symington 2022, Wagner 2020, Agwu 2020, Tuthill2019, Kahlert 2018) Se a carga viral não estiver suprimida, recomenda-se a extração e pasteurização do próprio leite ou leite de doadora pasteurizado (leite de banco) ou fórmula comercial infantil até alcançar carga viral negativa. (Panel 2024, WHO 2021) Vários países desenvolvidos (Suíça, Alemanha, Grã-Bretanha, Estados Unidos, Itália), ainda recomendando evitar a LM como medida mais eficaz para prevenir a TMI do VIH, mudaram suas diretrizes para apoiar pessoas soropositivas que decidem amamentar, recebem TARc e têm carga viral indetectável sustentada. (HHS Panel 2023, Gilleece 2019, Kahlert 2018) Nesses países, dezenas de mães com VIH (25 na Suíça, 44 nos Estados Unidos, 28 no Canadá. 30 na Alemanha, 13 na Itália, ) amamentaram, e a detecção do VIH nos bebês que mamavam foi negativa meses depois de finalizada a amamentação . ( Crisinel 2023, Levison 2023, Weiss 2022, Prestileo 2022) São necessárias políticas que maximizem o percentual de diagnóstico de pessoas infectadas por VIH e a profilaxia em casos sorodiscordantes (mãe soronegativa com parceiro VIH +) para diminuir os casos de TMI que ocorrem quando a mãe é contagiada durante a amamentação. (Anderson 2020, Tan 2017, WHO 2017 y 2014) A combinação mais utilizada de TAR é a associação de dois Inibidores da Transcriptase Reversa Análogo Nucleosídeo (ITIAN) com um Inibidor da Protease reforçado com ritonavir (IP/r) ou um Inibidor da Integrase (II): 2 ITIAN + 1 IP/r ou 1 II. (HSJD-HC 2023, Nelson 2014) Durante a gravidez e o pós-parto, os regimes de TARc que incluem dolutegravir (especialmente em associação com emtricitabina e tenofovir alafenamida) parecem mais seguros e eficazes no controle da carga viral do que os que não o incluem (Chinula 2023) , podendo inclusive associá-lo unicamente à lamivudina. (Rohr 2024) Os TAR são excretados em quantidades muito baixas na LM, com uma dose relativa inferior a 10%, e são muito bem tolerados pelos bebês que mamam (Aebi-Popp 2022, Kapito 2021, Mugwanya 2017 y 2016, Palombi 2016 ) . Pode surgir preocupação pelo possível desenvolvimento de resistência se ocorrer infecção pelo VIH no bebê que mama.
