Uma intervenção cirúrgica, seja com anestesia geral, epidural, regional ou local, não contraindica a amamentação. A mãe pode dar o peito até antes de entrar em cirurgia e pode voltar a fazê-lo assim que acorde da anestesia. A maioria dos anestésicos se excretam em pequena concentração no leite e/ou são de eliminação muito rápida, o que permite amamentar assim que a mãe esteja acordada, alerta e se encontre em condições e capaz de sustentar seu bebê (Mitchell 2020, Oliveira 2019, Fischer 2019, Reece 2017, Cobb 2015, Dalal 2014, Kranke 2011, Sønderskov 2011, Dahl 2011, Nitsun 2006, Lang 2003, Lee 1993). A pequena quantidade de colostro obtida pelo recém-nascido nas primeiras mamadas torna ainda mais supérfluo o esperar a amamentar após uma cesárea. O início precoce da alimentação da mãe após despertar da anestesia melhora as condições para a amamentação (Al-Takroni 1999). A cirurgia e a anestesia aumentam os níveis de prolactina (Lawrence 2016 p65 e p606, Morgan 1976). Se vai durar muitas horas a intervenção, pode-se extrair previamente leite, e inclusive enquanto dura a intervenção, para evitar incômodos. As intervenções plásticas sobre a mama podem posteriormente diminuir a produção do leite (Lawrence 2016 p231).
