Comentário
O iodo radioativo concentra-se na tireoide e no leite materno e pode suprimir a função tiroidea do lactente. O I-123 utiliza-se como agente de diagnóstico. Sua meia-vida física (radioativa) é de 13,2 horas (Howe 2008) e a meia-vida de eliminação biológica de 4,8 a 10,2 horas (Stabin 2000). Após 10 meias-vidas físicas (66 horas) o elemento deixa de ter radioatividade significativa e após 5 meias-vidas biológicas (51 horas) sua concentração no leite é imperceptível. Se quiser limitar a exposição a 0 milisievert (mSv) há que interromper a amamentação durante 5 a 10 meias-vidas biológicas. É possível também determinar a radioatividade em amostras de leite. Dado que a radiação natural anual que recebemos é de cerca de 6 mSv uma atitude mais conservadora é esperar apenas o tempo necessário para que o lactente esteja exposto a menos de 1 mSv (0,1 rem). O iodo-123 administrado a mães lactantes deve ser puro e não conter restos de I-124 ou I-125 (Mitchell 2019). Tempos de interrupção de amamentação que garantem uma exposição menor de 1 mSv: I-123 como iodohipurato ou ortoidodohipurato de sódio (123 I-OIH): Doses até 100 MBq (4 mCi) esperar 8 a 12 horas. Comissão reguladora nuclear dos Estados Unidos: não é preciso interromper a amamentação. I-123 como iodeto sódico (123 I-NaI). Se não contaminado com I-125: Doses até 20 MBq (0,5 mCi) esperar 3 a 6 horas. Se contaminado, segundo a ICRP (Mattsson 2015, Sachs 2013, ICRP 2008): mais de 3 semanas, segundo a American Thyroid Association (Alexander 2017 Q91 R77): 3 a 4 dias. I-123 como Iobenguano (Y-123 metaiodobenzilguadina, I-123-MIBG). Se não contaminado, interromper a amamentação 12, 24 ou 48 horas segundo a dose seja de 150, 370 ou 400 MBq (4, 10 ou 11 mCi). Com contaminação de I-124 ou I-125, esperar mais de 3 semanas (Sachs 2013, ICRP 2008). I-123 como ioflupano: Interromper a amamentação 1 dia (Sociedade de medicina nuclear, Djang 2012) a 6 dias (segundo o fabricante). Seroalbúmina humana marcada com I 123 (123 I-HSA): > 3 semanas (Sachs 2013, ICRP 2008). I 123-ácido iodofíltico (ácido beta-metil-15-(4-iodofenil) pentadecanoico; I 123 BMIPP): > 3 semanas (Sachs 2013, ICRP 2008). I 123-ácido fenilpentadecanoico (123 I-IPPA): > 3 semanas (Sachs 2013, ICRP 2008). Para maior segurança, pode-se solicitar medir a quantidade de radioatividade do leite no serviço de medicina nuclear. Se quiser continuar a amamentação há que prever extração e conservação de leite materno em congelador nos dias ou semanas prévios. Os dias aconselhados de interrupção de amamentação após a prova pode-se alimentar de forma exclusiva ou mista com o leite armazenado previamente. Após a prova segue-se extraindo leite que pode ser armazenado em congelador aparte durante 10 meias-vidas físicas (132 horas = 5,5 dias, após os quais a radiação terá desaparecido e poderá voltar a empregar-se. As mães que recebem uma dose inferior a 400 MBq para uma exploração da tireoide não é necessário que se abstenham de contato estreito com seus filhos.
