A meia-vida física é de 192 horas (8 dias), mas a efetiva está em função da meia-vida de eliminação biológica (VmB) de cada composto de I-131. O I-131 Na (de sódio) utiliza-se para tratamento ou exploração da tireoide. Sua meia-vida efetiva é de 11 horas. Acumula-se na tireoide e no leite. Se a dose administrada é maior de 1 mCi é necessário suspender a amamentação (ARSAC 2019). Além disso é necessário esperar a administrar o I-131 Na de 4 (Mitchell 2019, Sachs 2013) a melhor 6 semanas (ARSAC 2020, SNMMI 2018 e 2011, Am.Thy.Asoc. 2011) após ter suspendido a amamentação, devido a que se acumula muito na mama que amamenta e pode danificá-la. Se a dose administrada de I-131Na é menor de 1 mCi (37 MBq) se pode reanudar a amamentação após 41 dias de suspensão, o que não é prático nem fácil. O I-131 OIH (orto-iodohipurato sódico) utiliza-se para o diagnóstico. Sua meia-vida efetiva é de 6 horas. Com 30 horas de suspensão de amamentação não há risco de irradiação ao bebê que mama. Em mães que amamentam recomenda-se fazer a exploração de tireoide com I-123 e melhor com Tecnécio, radionuclídeos de meia-vida mais curta. Se se quer continuar a amamentação é necessário prever extração e conservação em congelador em dias ou semanas anteriores. Os dias aconselhados de interrupção de amamentação após a prova se pode alimentar de forma exclusiva ou mista com o leite armazenado previamente. Após a prova se segue extraindo leite que se pode descartar já que não é muito prático armazenar em congelador aparte durante 10 meias-vidas físicas (¡80 dias!), após as quais a radiação terá desaparecido e se poderá voltar a empregar. É possível também determinar a radioatividade em amostras de leite. Tentar empregar o radionuclídeo de meia-vida mais curta.
