Comentário
Antirretroviral inibidor da transcriptase reversa análogo de nucleotídeo (ITIN), com atividade antiviral contra o HIV-1 e o vírus da hepatite B. Indicado no tratamento da infecção por HIV e AIDS e na hepatite B crônica. Administração junto a outros antirretrovirais por via oral em uma ou duas doses diárias. Embora os níveis no leite materno sejam superiores aos plasmáticos (Rezk 2008 ), alcança baixa concentração, clinicamente não significativa, no leite materno. (Aebi 2022, Ramírez 2018, Waitt 2018 y 2017, Palombi 2016 y 2012, Corbett 2014, Shapiro 2013 y 2005, Pirillo 2015, Mirochnick 2009, Giuliano 2007, Moodley 1998) Não foram observados problemas em lactentes cujas mães a tomavam. (Kobbe 2016, Palombi 2012, Bae 2008) Os níveis plasmáticos desses lactentes foram indetectáveis ou muito baixos, menores que 5% - 10% dos níveis plasmáticos maternos. (Aebi 2022, Waitt 2018 y 2017, Palombi 2016, Kobbe 2016, Pirillo 2015, Corbett 2014, Shapiro 2013 y 2005, Palombi 2012, Mirochnick 2009) Pode ser preocupante o aparecimento de resistências ao tratamento devido aos níveis subterapêuticos recebidos pelos lactentes. (Inzaule 2016) Uso autorizado em recém-nascidos e lactentes. Diversas sociedades médicas e autores especialistas consideram seguro o uso desta medicação durante a amamentação.
