Comentário

Antiviral inibidor da protease (IP) do VIH. É utilizado, potencializado com ritonavir, no tratamento do VIH/SIDA. Foi empregado de modo experimental no tratamento do coronavírus COVID-19 (Li 2020, Lim 2020, SEFH 2020) . Administração oral em duas doses ao dia. Os dados farmacocinéticos do lopinavir (peso molecular moderadamente elevado e porcentagem muito alta de ligação a proteínas) explicam a passagem ínfima ou nula para o leite observada. (Oumar 2018, Ramírez 2018, Corbett 2014, Shapiro 2013, Palombi 2012, Rezk 2008) Os níveis plasmáticos em lactentes cujas mães tomavam lopinavir foram indetectáveis (Oumar 2018, Corbett 2014, Gandhi 2013, Shapiro 2013) ou muito baixos. Segundo a política adotada em cada país, a amamentação em mães com VIH / SIDA é ou não aconselhada. Com o tratamento antirretroviral combinado atual, a carga viral para VIH pode se tornar indetectável, reduzindo o risco de transmissão de VIH através da amamentação a níveis semelhantes aos da alimentação com fórmula artificial.
