Comentário
É um anticorpo monoclonal IgG1 kappa anti-IL-5 totalmente humanizado que é usado junto com outras terapias para tratar asma grave, granulomatose eosinofílica com poliangeíte e síndrome hipereosinofílica. Não deve ser usado para tratar uma crise aguda de asma. Administração subcutânea uma vez a cada 4 semanas. Na data da última atualização não encontramos dados publicados sobre sua excreção no leite materno. Seu elevadíssimo peso molecular (>100.000 Da) torna muito improvável a passagem para o leite em quantidade significativa, já que moléculas de mais de 500-1.000 Da não passam para o leite materno. (Hale, Almas 2016, Anderson 2016) Comprovou-se a passagem nula ou ínfima para o leite materno de outros anticorpos monoclonais similares, como adalimumab, belimumab, certolizumab, golimumab, infliximab, ipilimumab, natalizumab, omalizumab, rituximab, tocilizumab e ustekinumab. (Bar-Gil 2021, LaHue 2020, Ciplea 2020, Saito 2020, 2019 e 2018, Krysko 2019, Whittam 2019, Klenske 2019, Matro 2018, Anderson 2018, Bragnes 2017, Witzel 2014, Ross 2014, Fritzsche 2012) Por sua natureza proteica é inativada no trato gastrointestinal, não sendo absorvida (biodisponibilidade oral praticamente nula), o que dificulta ou impede a passagem para o plasma do lactente a partir do leite materno ingerido (Lactmed, Rademaker 2018, Bragnes 2017, Götestam 2016, Witzel 2014, Butler 2014, Mervic 2014), exceto em prematuros e período neonatal imediato, nos quais pode haver maior permeabilidade intestinal. (Sammaritano 2020) Não foram detectados problemas em lactentes cujas mães lhes administravam outros anticorpos monoclonais similares, como belimumab, bevacizumab, infliximab, omalizumab, rituximab, tocilizumab ou ustekinumab. (Bar-Gil 2021, LaHue 2020, Saito 2020, 2019 e 2018, Klenske 2019, Mugheddu 2019, Krysko 2019, Matro 2018, Bragnes 2017, Hyrich 2014, Danve 2014) Autores especialistas consideram seguro ou muito provavelmente seguro o uso deste (Middleton 2020) ou outros anticorpos monoclonais durante a amamentação. (Middleton 2020, Whittam 2019, Matro 2018, Anderson 2018 e 2016, Witzel 2014, Pistilli 2013) Dada a forte evidência que existe sobre os benefícios da amamentação materna para o desenvolvimento dos bebés e a saúde das mães, convém avaliar o risco-benefício de qualquer tratamento materno, incluída a quimioterapia, aconselhando individualmente cada mãe que deseje seguir com a amamentação.
