Comentário
Estimulante central e simpaticomimético indireto com ações e indicações semelhantes às da dexanfetamina. Indicado no transtorno de déficit de atenção com hiperatividade (TDAH) e na narcolepsia. Administração oral 1 a 3 vezes ao dia. É excretado no leite materno em quantidade clinicamente não significativa ou nula (Collin 2018, Bolea 2014, Spigset 2007, Hackett 2006) e não foram observados problemas a curto ou longo prazo em lactentes cujas mães o tomavam. (Collin 2018, Rowe 2013, Bolea 2014, Spigset 2007, Hackett 2006) Os níveis plasmáticos desses lactentes foram indetectáveis ou muito baixos. (Bolea 2014, Hackett 2006) Diversas sociedades médicas e especialistas consideram seguro o uso desta medicação durante a amamentação. (Calvo 2018, Collin 2018, Ornoy 2018, Marchese 2015, Rowe 2013) Algum autor sugere vigiar clinicamente o ganho de peso e possível irritabilidade no lactente. (Marchese 2015, Rowe 2013) O metilfenidato não influencia de maneira regular os níveis de prolactina (Janowsky 1978), mas foram descritos aumento da produção de leite (Bello 2022) e galactorreia. (Ekinci 2017)
