Olanzapina é antipsicótico atípico segunda geração, antimaníaco estabilizador do humor que usa-se no tratamento esquizofrenia e mania moderada a grave associada com transtorno bipolar junto com fluoxetina para tratamento fase depressiva do transtorno bipolar e depressão unipolar resistente ao tratamento. Administração oral uma vez ao dia. Administração intramuscular de ação prolongada cada 2 a 4 semanas. Excreta-se em leite materno em quantidade clinicamente não significativa (Manouilenko 2018, Aydin 2015, Stiegler 2014, Var 2013, Whitworth 2010, Lutz 2008, Ambresin 2004, Gardiner 2003, Croke 2002). Não observaram problemas significativos em bebês que mamam cujas mães tomavam, nem no desenvolvimento psicomotor nem linguagem, nem no ganho de peso (Uguz 2017 e 2016, Grover 2015, Stiegler 2014, Whitworth 2010, Lutz 2008, Gardiner 2003, Friedman 2003, Croke 2002, Kirchheiner 2000), ou diferenças com grupos controle não foram estatisticamente significativas (Gilad 2011). Níveis em plasma daqueles bebês foram indetectáveis ou muito baixos (Manouilenko 2018, Stiegler 2014, Whitworth 2010, Lutz 2008, Gardiner 2003, Croke 2002, Kirchheiner 2000). Publicaram-se casos isolados sedação (em associação com outra medicação) e de prisão ventre (Sinha 2021). Quando houve exposição pré-natal a olanzapina, 16% de recém-nascidos experimentaram algum efeito adverso tipo sonolência, irritabilidade, tremores ou insônia (Brunner 2013). Há menor frequência elevação de prolactina que com outros antipsicóticos (David 2000, Crawford 1997). Autores especialistas consideram seguro o uso de olanzapina durante amamentação (Uguz 2021 e 2016, Teodorescu 2020, Rybakowski 2019, Larsen 2015, Parikh 2014, Rowe 2013, Klinger 2013), sendo fármaco antipsicótico de primeira escolha na amamentação (Pacchiarotti 2016).
