A amamentação se associa à perda óssea transitória, com recuperação completa da densidade mineral óssea (DMO) dentro de 6 - 12 meses após o desmame, o que minimiza qualquer dano significativo ao esqueleto materno. (Capozzi 2021, Oboh 2021, Grizzo 2020, Sowers 1993) A relação da amamentação e sua duração com a osteoporose e o risco de fratura (de quadril ou outras) pós-menopáusicas é muito controversa, pois enquanto há autores e revisões sistemáticas que não encontram nenhuma associação (Bakker 2020, Yan 2019, Abou 2018, Crandall 2017, Mori 2015, Chowdhury 2015, Lebel 2014, Kovacs 2014, Yumusakhuylu 2013, Lenora 2009, Ip 2007 ), outros estudos encontram que a amamentação (e mais se for prolongada) é um fator protetor contra osteoporose e fratura de quadril pós-menopáusica (Crandall 2017, Cooke 2017, Jiménez 2015, Bjørnerem 2011, Sadat 2005), e outros autores encontram associação entre amamentação prolongada e DMO baixa, com maior prevalência de osteoporose e fraturas (não especialmente de quadril) após a menopausa. (Yoo 2021, Lee 2020 y 2019, Hwang 2016, Kim 2015, Tsvetov 2014, Bolzetta 2014, Okyay 2013) O diagnóstico de osteoporose durante a gravidez ou a amamentação não contraindica a amamentação. É importante uma ingestão adequada de cálcio e vitamina D durante a gravidez e a amamentação, mediante dieta equilibrada e exercício regular e com ajuda de suplementos dietéticos se necessário. A maioria dos tratamentos para osteoporose é compatível com a amamentação. Apenas na doença infrequente conhecida como osteoporose associada à gravidez e à amamentação está indicado suprimir a amamentação.
