Para recém-nascidos prematuros (nascidos antes semana 37 de gestação), leite própria mãe é alimento referência para prevenir doenças curto (enterocolite necrotizante, infecções neonatais) longo prazo. Unidades neonatais devem adotar enfoques que facilitem alimentação com leite própria mãe (Schanler 1995). Quando não é possível leite própria mãe, leite banco doado é melhor opção para saúde do prematuro que fórmulas lácteas comerciais (Lawrence 2016). Muitos prematuros, em especial os nascidos mais pré-termo menor peso vão necessitar cuidados hospitalares em unidades neonatais não vão poder alimentar-se diretamente do peito: precisarão leite extraído manualmente ou com bombas extractoras ("sacaleches") pela própria mãe administrada por meio sonda naso ou oro-gástrica. Administração temprana colostro, método de cuidados mãe-canguru e "cuidados centrados no desenvolvimento família" são indispensáveis para lograr óptimo resultado saúde do prematuro.
