Comentário

Quilotórax é extravasação quilo ou linfa sistema linfático ao espaço pleural. Primeiros meses vida ocorre após cirurgia malformações congênitas (cardiopatia, atresia esôfago, hérnia diafragmática) ou modo congênito, associado ou não outras malformações, cromossômicas frequência, como trissomia 21 (Samanidis 2022, Medoff 2010, Lumbreras 2009, Prada 2008, Yamamoto 1996). Tratamentos conservadores tentam reduzir ao mínimo ou suprimir fluxo linfático obter fechamento defeito; consistem em jejum absoluto nutrição parenteral, dieta livre ácidos graxos cadeia longa rica triglicerídios cadeia média (MCT), octreotídio (análogo somatostatina) e suporte respiratório drenagem pleural quando requerido (Martínez 2002, Kankananarachchi 2021). Cirurgia reparadora é opção difícil quando anterior falhou. Percentual importante pacientes melhoram com fórmulas especiais livres ácidos graxos cadeia longa ricas MCT (Biewer 2010, Lumbreras 2009). Provou-se com êxito leite materno desnatado obtido após centrifugação enriquecido ou não com MCT (Clark 2023, Höck 2021, DiLauro 2020, Sánchez 2020, Neumann 2020, Concheiro 2019, Fogg 2016, Kocel 2016, Chan 2007, Hamdan 2004, Trish 2003). Há diversos métodos efetivos simples eliminar gordura leite materno (Barbas 2020, Drewniak 2013). Obtenção leite materno desnatado por meio centrifugadora no domicílio familiar foi demonstrada factível (Huang 2024). Existência componentes imunitários protetores no leite materno supõe alimentação ótima muito superior alimentação com fórmula comercial facilita voltar amamentação administrada diretamente peito materno (Höck 2021). Preparação leite humano baixa gordura diminui conteúdo IgA proteínas. Além gordura (forma MCT), provável que bebês que mamam alimentados com leite humano baixa gordura necessitem suplemento proteínas favorecer crescimento adequado (Drewniak 2018). Pode precisar-se jejum total nutrição parenteral. Octreotídio ou somatostatina, diminuem absorção intestinal, são também outra opção (Costa 2018, Lumbreras 2009, Prada 2008, Roehr 2005, Hamdan 2004, Rimensberger 1998). Média dias introdução dieta normal são 9 dias (variação 3 a 59) que convém saber se deseja conservar amamentação meio extração regular congelamento
