Comentário

Antirretroviral inibidor da integrase. Indicado em combinação com outros dois fármacos antirretrovirais no tratamento da infecção pelo vírus da imunodeficiência humana (VIH 1) e na profilaxia da transmissão vertical (gravidez/amamentação) (Puthanakit 2018, Westling 2012) e pós-exposição acidental. Administração oral em 2 doses diárias. É excretado no leite materno em quantidade clinicamente não significativa e os níveis no plasma dos lactentes foram muito baixos ou abaixo do limite de detecção. (Aebi 2022, Feiterna 2020) É um fármaco bem tolerado e com baixa frequência de reações adversas graves (MSD 2019, AEMPS2018, Clarke 2018, Ribera 2010), incluindo neonatos, lactentes e crianças (Clarke 2018, Ripamonti 2016, Nachman 2015 y 2014). É utilizado em recém-nascidos não prematuros e a partir de 3 Kg de peso (Trahan 2019, MSD 2019, AEMPS 2018). A OMS contempla o uso neonatal (Clarke 2018, WHO 2016, WHO 2014). É recomendado a partir de 2 a 4 semanas de vida nos esquemas de profilaxia pós-exposição não ocupacional ao VIH (CDC 2016, WHO 2014). Nos recém-nascidos prematuros, tem semivida de eliminação mais longa, entre 15 e 22 horas (Kreutzwiser 2017). "Individuals with HIV who are on ART with a sustained undetectable viral load and who choose to breastfeed should be supported in this decision".
