Comentário
Antibiótico bactericida do grupo das rifamicinas. Utilizado para tratar diversas infecções bacterianas, mas sobretudo tuberculose, outras infecções por micobactérias, lepra, legionella e pneumococos resistentes à penicilina. É utilizado na profilaxia de várias meningites graves. Administração oral ou intravenosa. É excretada no leite materno em quantidade clinicamente não significativa (Kawuma 2024, Partosch 2018, Vorherr 1974, Lenzi 1969), muito inferior à dose empregada no tratamento ou profilaxia de infecções em neonatos e lactentes. A dose relativa em diversos trabalhos oscila entre 0,4% e 3,7%, salvo em um estudo antigo e com falta de detalhes (Lenzi 1969), no qual superava 5%. Somente em um lactente de seis cujas mães tomavam rifampicina foram detectados níveis plasmáticos de rifampicina. (Kawuma 2024) Não foram observados problemas atribuíveis à rifampicina em lactentes cujas mães a tomavam. (Ozturk 2017, Peters 2008, Keskin 2008, Drobac 2005) Pode tingir líquidos corporais, incluindo o leite, de cor vermelho-alaranjada. (Anderson 2018) Diversas sociedades médicas e autores especialistas consideram seguro o uso desta medicação durante a amamentação. (Hale, LactMed, Algharably 2023, Briggs 2015, Baquero 2015, Butler 2014, Mitrano 2009, CDC 2003, ATS 2003, Tran 1998, Snider 1984) Academia Americana de Pediatria: medicação usualmente compatível com a amamentação (AAP 2001).
