Leitura clínica

Comentário

Macrolídeo imunossupressor por inibição da calcineurina, utilizado para evitar a rejeição de órgãos transplantados (rim, fígado, coração, etc.) e no tratamento do eczema atópico, psoríase e outras doenças autoimunes (artrite, lúpus, doença inflamatória intestinal...). Administração sistêmica oral e tópica dermatológica. Seu elevado peso molecular e alta ligação a proteínas explicam a excreção clinicamente insignificante obtida em colostro e leite materno. (Kuczaj 2024, Akamine 2021, Hiramatsu 2018, Kociszewska 2018, Zheng 2013, Bramham 2013, Gardiner 2006, French 2003, Jain 1997) Não foram observados problemas de desenvolvimento nem imunológicos, a curto ou longo prazo, em mais de 200 lactentes cujas mães o tomavam (C oscia 2025, Deans 2025, Kuczaj 2024, Mizutani 2023, Boyle 2021, Klintmalm 2020, Nure 2019, Constantinescu 2014, Izumi 2014, Thiagarajan 2013, Gouraud 2012, French 2003), salvo um caso leve e autolimitado de trombocitose transitória. (Gouraud 2012) Os níveis no plasma desses lactentes foram indetectáveis ou muito baixos. (Kuczaj 2024, Akamine 2021, Hiramatsu 2018, Izumi 2014 Bramham 2013, Gomez-Lobo 2012, Gouraud 2012) Sua baixa biodisponibilidade oral, ainda mais reduzida quando coincide com a ingestão de alimento (EMA 2015), especialmente se este for gorduroso, dificulta a passagem para o plasma do lactente a partir do leite materno ingerido. Os lactentes que foram expostos a inibidores da calcineurina durante a gravidez podem ter deficiência imunológica no primeiro ano de vida (Grimer 2007), daí a importância de não serem privados da amamentação sem um motivo demonstrado. O Tacrolimús pode provocar uma leve diminuição dos níveis séricos de prolactina, que não tem repercussão na produção de leite materno em uma amamentação já estabelecida. ( Szypulska 2021) Diversas sociedades médicas e consensos de especialistas consideram seguro o uso de Tacrolimús durante a amamentação.
