O Tecnécio 99 metaestável (Tc 99m) é um isótopo radioativo que emite radiação gama. Sua meia-vida de desintegração radioativa é de 6,0 horas. Após marcação radioativa com uma solução de pertecnetato (99mTc) de sódio, a solução de pentetato de tecnécio (99mTc) obtida é utilizada em cintilografia para o diagnóstico de doença renal, cerebral, pulmonar ou gastroesofágica mediante administração intravenosa, inalada ou oral. Entre 0,01% e 0,2% da dose administrada é excretada no leite materno (Leide 2016, Liepe 2016). Os tempos de interrupção da amamentação ou de evitar contato próximo calculam-se para que o lactente não fique exposto a mais de 1 milisievert (1 mSv = 0,1 rem) de radiação (ARSAC 2019, US.NCR 2016, ICRP 2008, Stabin 2000). Um adulto recebe entre 5 e 10 mSv anuais procedentes da radiação ambiental. Os principais organismos reguladores de administração de substâncias radioativas e especialistas consideram que se pode amamentar imediatamente após um teste diagnóstico com Tc 99m Pentetato (Mitchell 2019, ARSAC 2019 p51, US.NCR 2016 tab U.3, ICRP 2008 p163, Stabin 2000). Alguns (ARSAC 2019, ICRP 2008, Ahlgren 1985, Mountford 1985 e 1984) consideram mais prudente interromper a amamentação durante cerca de 4 horas, extraindo uma vez o leite e podendo oferecer, em seu lugar, leite previamente extraído e armazenado na geladeira antes do teste. Se a dose administrada foi ≥ 800 MBq recomenda-se uma interrupção de 5 horas (ARSAC 2019 p51). O leite extraído após o teste pode ser congelado e empregado uma vez passadas 10 meias-vidas radioativas: 10 x 6,0 = 60 horas = 3 dias (Hale 2017, p. 2019).
