Vitamina B12 existe naturalmente no leite em concentração 1 nanograma/mL (Sauberan 2019; intervalo: 0,3 a 3 ng/mL). Suplementação materna com cianocobalamina apenas aumenta níveis no leite mulheres bem nutridas (Sandberg 1981) mas sim que os melhora na de mulheres baixo nível socioeconômico (Sneed 1981). Concentração colostro é até 28 vezes superior à leite maduro. Necessidades diárias Vitamina B12 são 2,4 microgramas (Sauberan 2019) e aumentam 2,8 microgramas diários durante amamentação. Alimentos ricos vitamina B12 são carnes, vísceras, ovos, lácteos, salmão, sardinhas, amêijoas e cereais enriquecidos. Com dieta variada e equilibrada não se necessitam suplementos vitaminas, mas foi-se observado carência B12 em plasma e leite mulheres com dietas vegetarianas estrictas (veganas), doenças má absorção (p.e: Crohn, doença celíaca), cirurgia bariátrica, mal estado nutricional, anemia perniciosa, nível socioeconômico baixo e em caso catástrofes naturais ou guerras. Há numerosas publicações que demonstram que lactentes destas mães têm alto risco doença por carência vitamina B12 que pode cursar com anemia, atraso crescimento e atraso psicomotor. A "pasteurização Flash" aplicada leite mães HIV + não diminui concentração vitamina B12. Academia Americana Pediatria: medicação usualmente compatível com amamentação (AAP 2001).
